Verification of Employment
How do I get verification of employment?
For any verification of employment for state of Utah employees, please contact the Employee Resource Information Center (ERIC) at 801.957.9390 or you can fax us at 385.465.6054, or email to erictransactions@utah.gov.
In order for the ERIC to complete a written verification of employment for an employee, they must have at least the last four digits of the employee’s social security number, and the request must be accompanied by an authorization form that is signed by the employee. Information we can provide in a written verification of employment:
- Dates of Employment
- Salary (including hourly rate, current and prior yearly salary)
- Average hours worked
- Date of last pay increase
- Amount of last pay increase
