Proposed change to new hire/rehire date
DHRM needs to notify payroll about a proposed change to a new hire or rehire date that is not already in payroll. DHRM can do this by sending an email to payroll@utah.gov detailing the information that has changed.
W2 for retiree's
If you are retired, and have received a W2 from the state with only an amount in box 12A, this is not reporting that you made income. Box 12A is showing what the state paid for your health insurance for the year. This is for your records.
FICA exempt
Non resident alien/here on a student visa or if your social security card is restricted and you should be FICA exempt, please make sure you send all INS paperwork to payroll@utah.gov. If you have further questions please open a ticket.
Name Change
If you have had a legal name change and need your name updated in the payroll system please email a copy of your new social security card with your updated name on it to payroll@utah.gov or fax it to 385-465-6012
Change of Address
